HQC will take reasonable steps to maintain availability of Transform My Practice and ensure the application contains reasonable safeguards to protect the accuracy and integrity of the data. Use of information contained on this website is in no way intended to replace or be a substitute for professional judgement.

HQC (including its directors, officers, employees, or contract personnel) provides the web site and information contained within the web site on an "as is, where is" basis and does not make any express or implied warranties, representations, endorsement, or conditions with respect to the website of information contained within the website, including without limitation, warranties as to the merchantability, operation, non-infringement, usefulness, completeness, accuracy, currentness, reliability, and fitness for a particular purpose.
In no event will HQC or their employees and contractors be liable for any special indirect of consequential damages regardless of whiter the action for such damages is brought in tort, including without limitation negligence and contract including without limitation fundamental breach.
Use of Your Information
Your contact information will only be used to verify your clinic location and to follow-up with you regarding any further support we can provide. This helps us provide the best support possible and can help you to get the most value from the data you enter. Your clinic's data and contact information will not be shared with anyone without your permission.
The CPR™ Initiative is responsible to inform its stakeholders on its progress and results. One way is to report the combined results of the clinics’ measurements. There will be no identifying clinic or provider information. Data that is entered is aggregated - no individual clinic level data is identified in provincial reports. It is possible that similar clinic groups may be graphed together. For example: information about similar specialties may be grouped together or providers may be linked geographically.
The provincial reporting categories will not be available for display for comparison against clinic level data unless three or more clinics are reporting data in the reporting category.
The following is an example of how the data would be reported.
Note: Lines 1-5 may represent geographical groups (such as region or community), individual practice (with no identifying information), or peer groups (such as similar specialties). Each individual practice can also generate its own practice level reports.
Termination of Participation
Both the practice and HQC have the right to terminate participation in the CPR™ initiative for any reason, including the ending of coaching services that are already underway. If this commitment is terminated, the parties shall be liable only for performance rendered prior to the effective date of termination. Practice level data will remain in the database and be available through this website up to the termination of participation in the CPR™ initiative and beyond if the clinic chooses to collect data outside the CPR™ initiative. All practice level data collected in this website prior to termination of participation will continue to be included in HQC generated provincial reporting.
General Terms and Conditions
Any personal information that you voluntarily provide through the completion of electronic forms on this web site is collected and maintained in compliance with the Local Authority Freedom of Information and Protection of Privacy Act. Personal information collected is used only for the purpose(s) for which it was collected, and the HQC does not disclose this information to other organizations or individuals, except as authorized by law.
